About Facelifts

One of the best ways to improve your vivaciousness is with a facelift performed by double board-certified plastic surgeon Dr. Paul S. Gill, which is a procedure that corrects aging and droopiness of the jaw, neck, and lower face. Dr. Gill performs a full spectrum of facial surgical treatments in The Woodlands, TX at The Gill Center for Plastic Surgery and Dermatology. The effects of time on the lower regions of the face can be harsh, resulting in drooping jowls and unsightly folds. With a facelift performed by Dr. Gill, you can see that excess drooping skin vanishes through a combination of skin and tissue tightening techniques that will erase some signs of advanced aging in the lower face. Traditionally, the facelift is one of the most sought-after surgeries at The Gill Center for Plastic Surgery and Dermatology. This surgery focuses on distinct areas of the face to tighten loose skin and wrinkles to create a tauter, yet softer appearance. Based upon your concerns and goals for surgery, a facelift can focus on the lines and wrinkles near the eyes, the mid-face area and cheeks, the bottom two-thirds, including the mouth, jowls, neck, and more for an all-encompassing younger look.

An excellent candidate will be a patient with platysma banding, loss of the cervical mental angle, jowling, or nasolabial folds. Dr. Gill is a highly trained and experienced surgeon who understands the complexity and delicate anatomy of a facelift procedure. The overall goal is to produce a natural appearance that's gorgeous, subtle, and still striking. Patients typically present to The Gill Center inquiring about a facelift after aging and sun damage has caused laxity in the neck and the jawline. Dr. Gill regularly consults with patients wanting the correction of the platysma muscle, which forms bands, fiber crossing from one side to the other (decussation), and herniation of some of the underlying fat tissue. They also have loss of definition of their jawline with significant jowling. If you would like to set up an appointment with Dr. Gill, contact our office to book your consultation today.

Ideal Candidates

Optimal results from a facelift in Houston can have you looking more than 10 years younger, leading to improved quality of life following surgery. Most often, however, the goal of a facelift is to correct some of the following issues on the face as a whole or on the upper, mid, or lower face:

  • Sagging jowls
  • Excess skin in the lower face
  • Folds of skin on the neck
  • Loose underlying tissues on the lower face

To be an ideal patient for a facelift, you should be between 40 – 60 years old with good skin elasticity and be a healthy, non-smoker. You must undergo a thorough pre-operative medical workup to ensure that you are healthy enough for this procedure and a safe recovery. It is also important to have realistic expectations for the outcome of your procedure and to notify Dr. Gill if you have high blood pressure, blood clotting issues, or if you tend to form excessive scars.

Surgical Technique

In The Woodlands and North Houston area, we regularly perform a lateral SMASectomy facelift. This procedure focuses on muscle rejuvenation. The loose SMAS muscle, or the cheek and jawline, are removed and re-sutured to allow for tightening and correction of the area. Subsequently, we focus attention on the platysma muscle where we tighten this and secure it to the mastoid or mastoid bone. This allows for a secure correction of the neckline and treatment of neck laxity. Dr. Gill strongly believes in extensive muscle repair in the facelift to allow for a natural and long-lasting result. If the muscle is not correctly repaired and excess tension is placed on the skin, an unnatural and short-lived result will occur; therefore, after focusing on your muscle repair, Dr. Gill simply removes whatever skin is loose through an incision within the hairline and around the ear. Depending on subsequent procedures and the amount of neck laxity and soft tissue involvement, a facelift can take anywhere from 2 ½ – 4 ½ hours.

No matter the issue you wish to correct, there are various levels of facelifts to think about. Some facelifts address the upper area of the face and others focus on the lower two-thirds. In addition, some methods are a bit more invasive than others. It's very common for some patients to have an eyelid lift, brow lift, or BOTOX and fillers to complement and enhance their final results. Some of the other known facelift options include:

  • Full Facelift
A common surgery for older adults, the full or "traditional" facelift most often puts an incision close to the hairline, extending down to the area near the temples, stopping near the base of the ears. The deeper layer of facial muscles is then carefully stretched and safely tightened to create a lifted outcome, which reduces excessive loose skin and droopy features. This facelift is used for individuals who have visible signs of aggressive aging. During a full facelift, various procedures may be used, including blepharoplasty to address the eyes or neck reduction for the lower region.
  • Mid-Facelift
With a similar approach to the mini facelift, the surgery known as a mid-facelift can address the hollow and lax areas close to the cheekbones, as well as the upper jaw. The mid-facelift works well for those who hope to attain improvement in the middle to lower region of the face, which also improves the look of the jowls.
  • Lower Facelift
This lift corrects the chin area and neck and sometimes the jowls. Methods of a nonsurgical nature, such as a Kybella injection, can also be applied for chin augmentation to attain a younger, tighter-looking neck.
  • Mini Facelift
When undergoing a mini facelift, a thin, small incision is placed near the ears to lift the skin to a higher position, which enhances the mouth, jawline, and jowls. This process creates nice and subtle enhancements that look beautiful and natural.

What to Expect

Following your procedure, we will monitor your health until you can be safely released to be taken home by a friend or family member. Prior to your procedure, Dr. Gill or someone on his staff will schedule your follow-up visits and go over suggestions to help you heal properly. In some cases, a facelift can have a lengthy recovery, lasting up to three weeks. During this time, you will need to get plenty of rest and limit your activity. You may experience swelling and discomfort, which may be managed with medication. The scars you see will fade over time and Dr. Gill will let you know when you can apply mineral makeup to cover the after-effects of surgery.

A facelift procedure typically results in bruising and swelling that can last approximately two weeks. We utilize various supplements, such as Arnica, Bromelain, multivitamins, and healing packs, to assist in your recovery.  You should feel and physically look better after several weeks. Then, you can expect a more glowing look with fewer lines and less sagging. Overall, your face will appear lifted and younger looking.

Frequently Asked Questions

What is a facelift?
A facelift is a procedure designed to treat the three areas of the cheeks, the jawline, and the neckline. There are several different techniques we can utilize to address these concerns; however, they are based on the level of laxity the patient has on his or her face.

What are realistic expectations for a patient having a facelift?
Patients undergoing a facelift procedure must be realistic in regard to what can and cannot be altered by surgery. A facelift surgery is designed to adjust facial shape, not to handle fine lines or fine wrinkles. If the patient requests the removal of loose neck skin, jowls, and nasolabial folds, this can be adequately corrected with a combination of facial rejuvenation procedures.

How long does a facelift usually take to perform?
Depending on subsequent procedures and the amount of neck laxity and soft tissue involvement, a facelift can take anywhere from 2 ½ – 4 ½ hours.

Which technique is best for me?
The best technique for you will vary based on your concerns, goals, level of skin laxity, what areas are being addressed, and other factors. To produce your best results, Dr. Gill will go over the different techniques in your consultation. He can then help you choose which technique is best.

Should I get a surgical or nonsurgical facelift?
Although there are several great noninvasive skin tightening treatments, these can only give you small improvements. For moderate to severe skin sagging, facelift surgery is the best choice to improve your appearance. In your consultation, Dr. Gill will discuss your choices, including nonsurgical procedures, to help you select which is best for your goals.

What about wrinkle fillers and relaxers over facelift surgery?
Wrinkle fillers and relaxers, like BOTOX and Juvéderm, have become more popular to improve fine lines and wrinkles. Also known as a "liquid facelift," nonsurgical wrinkle treatments are good for early signs of aging; however, the results won't last more than 1 – 2 years. A facelift will produce more significant improvements than injectables with long-lasting results. Ask Dr. Gill about nonsurgical wrinkle treatments in your consultation.

Will my results look natural?
Some patients worry that a facelift will make their skin look shiny, overly tight, or like they are in a wind tunnel. Dr. Gill focuses on giving you results that look natural so you appear refreshed, not fake. He listens to your concerns and goals before creating your custom treatment plan to produce your best results.
